If you were born in the months January til April, click her for the ITC Registration form you will be registered in Dr. Lewis’s GP practice.

When you have completed your registration form, you can email it to: itcstudents@gcveldpoort.nl

If you were born in the months May til August, click here. You are referred to the practice of Dr. Snijders-Sijes.

If you were born in the months September til December, click here. You are referred to the practice of Dr.  Bonnema.
